Getting your Utah medical cannabis card shouldn’t be complicated.
We guide you through every step so you can focus on feeling better.

Initial Consultation – $175

This will be a in person 30 minute consultation with our provider to determine if you are a candidate for a cannabis card and to answer any of your questions.

Three month follow up – No Cost

This will be a phone call or telehealth visit with our provider to make sure everything is going ok and answer any questions.

Renewal visit – $100

This is a yearly visit with our provider, in person or telehealth to check in on things and renew the card.

Do I Qualify?

Not sure if you’re eligible for a Utah medical cannabis card? We make it easy to find out. If you meet a few basic requirements, you could be approved and on your way to relief in just days.

To qualify, you must:

  1. Must be a Utah Resident
  2. Must have at least one qualifying condition (see full list)
  3. Submit an application online through the state’s portal
  4. Meet in-person with a medical provider registered with the Utah Department of Health and Human Services to recommend medical cannabis
  5. The medical provider has certified your eligibility for a medical cannabis card online
  6. Pay a $15 application fee online

Qualifying conditions under the Utah medical cannabis act

  • HIV or acquired immune deficiency syndrome
  • Alzheimer’s disease
  • amyotrophic lateral sclerosis
  • cancer
  • cachexia
  • persistent nausea that is not significantly responsive to traditional treatment, except for nausea related to: pregnancy
  • cannabis-induced cyclical vomiting syndrome
  • cannabinoid hyperemesis syndrome
  • Crohn’s disease or ulcerative colitis
  • epilepsy or debilitating seizures
  • multiple sclerosis or persistent and debilitating muscle spasm
  • autism
  • a terminal illness when the patient’s life expectancy is less than six months
  • a condition resulting in the individual receiving hospice care
  • a rare condition or disease that affects less than 200,000 individuals in the U.S., as defined in federal law, and that is not adequately managed despite treatment attempts using conventional medications (other than opioids or opiates) or physical interventions
  • persistent pain lasting longer than two weeks that is not adequately managed, in the qualified medical provider’s opinion, despite treatment attempts using conventional medications other than opioids or opiates or physical interventions
  • acute pain that is expected to last for 2 weeks or longer for an acute condition, including a surgical procedure, for which a medical professional may generally prescribe opioids for a limited duration
